Athletic Information

                                                                              WINTER SPORTS   ONLY

 

 

Athletic Registration Due:  November 11, 2011

 

We encourage every Legend High School student to get involved in athletics and activities.  High School is a great time to participate because there are so many opportunities to develop skills in teamwork, good sportsmanship, self-discipline, and leadership.

 

Please register your student for the WINTER SPORTS ONLY at this time.

Registration and fees for the Spring seasons will be collected at a later time.

 

Cheerleading – Boys Basketball – Girls Basketball – Girls Swim and Dive -  Poms – Wrestling

 

• All Athletic Registration forms must be completed and payment for the $150.00 athletic participation fee must be paid before a student can practice or participate.  The winter sports season for Legend High School Titans begins on November 11, 2011 with the first authorized practice. 

 

• Athletic registration material may be turned into the Athletic Office beginning October 24, 2011

 

• Students must have a current physical on file.  Physicals are good for one year, 365 days

 

 

• The Athletic Office will only accept COMPLETED athletic forms and accompanying fees

Completed registration includes:

 

• Read and complete the ATHLETIC REGISTRATION/EMERGENCY INFORMATION form.  Parent or Guardian signature required. 

 

• Read and complete the Interscholastic Participant form.  Have physician sign and date the Physician Permit for Athletic Participation.

 

• Read and sign the Training Rules and Expectations / Parent or Guardian Advisement and Permit Form.  The form is two sided, please be sure to read and sign the back.

 

• Read and complete the DCSD Athletic/Activity Medical Information, front and back

 

• Read and sign Transportation Awareness Consent and Release form

 

• Please read and sign the Sporting Behavior form and the 10 things students want from spectators

 

• Attach $150.00 payment for the participation fee for each sport.  There is no family maximum this year.

 

PLEASE NOTE:  You will not be able to practice until all your athletic registration materials are completed, along with your athletic fee and returned to the athletic office.
