Fall Athletic Registration

* All athletic registration forms must be completed and paymentfor the $150.00 athletic participation fee must be paid before a student can practice or participate. Please turn all athletic forms into the Athletic Office. Fall Athletic Packets will be accepted after July 1, 2010. Please have all forms and athletic fee turned into the athletic office by the first day of practice. Please schedule your physicals early to avoid delaying athletic clearance.
* Students must have a current physical on file. Physicals are good for one year, 365 days.
* The Athletic Office will only accept COMPLETED athletic forms for FALL SPORTS and accompanying fee.
Completed registration includes:
1. Read and complete the ATHLETIC REGISTRATION/EMERGENCY INFORMATION form. Parent or Guardian signature required.
2. Read and complete the Interscholastic Participant form. Have physician sign and date the Physican Permit portion of the form.
3. Read and sign the Training Rules and Expectations/Parent or Guardian Advisement and Permit form. The form is two-sided, please be sure to read and sign the back.
4. Read and complete the DCSD Athletic/Activity Medical Information form, front and back.
5. Read and sign the Transportation Awareness Consent and Release form.
6. Please read and sign the Sporting Behavior form and read the Ten Things Students Want From Spectators.
7. Attach $150.00 payment.
